We can do all this by initiating an open conversation between the elderly adult and the adult family members to discuss concerns and wishes of those involved. The goal is to help manage the current situation, plan for the future and assist with the transitions from one stage to the next. Our services are offered on an as needed basis or may be a one time session or part of a long term individual care plan.
